Reporting Suspensions

1) All suspensions/incidents are to be reported according to District 4's  procedures which can be found at:  http://odmha.goalline.ca/page.php?page_id=32501 . You may do so on-line  or e-mail to  suspensions@district4.ca or by  Fax 1-888-250-8482

 
2) In addition, you must report all suspensions incurred in-league and out-of-league to your B League Division Convenor via email or fax (see below) http://obl.goalline.ca/page.php?page_id=32115 

Please include the following information:

• Your name and position with the team.
• The penalty received as shown on the game sheet, including:
  • date of the game;
  • type of game; league, playoff, exhibition or tournament game
  • player/official's name & number or position;
  • penalty/penalties assessed;
  • period and time;
  • other information (if necessary) - 1st fight, 2nd, 3rd, etc.,

• Dates the discipline (suspensions) will be served.

3) You must also copy your ORMHA, convenor for both the suspension and proof of dates served.

NOTE: since the gamesheets may not be the original version, set fax machine to scan with a "fine" setting so it will be as clear as possible for recipient.
